Function
Primary functions are to administer skilled nursing care, under the supervision of a registered nurse for clients of all ages in their pace of residence, coordinate care with the interdisciplinary team, patient/family and referring agency.

Job Qualifications
Education: Graduate of an accredited school of vocational Nursing.
Licensure: Current Missouri license as a Licensed Vocational Nurse, current Missouri drivers License.
Experience: Two years experience as a Licensed Vocational Nurse in a clinical care setting, home health preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of Current CPR and Hepatitis consent/declination.
Transportation: Reliable transportation required. Valid and current auto liability insurance.
Environmental and Working Conditions
Works in patient's homes in various conditions, possible exposure to blood, body fluids and infectious diseases; ability to travel locally; some exposure to unpleasant weather, PNR emergency call.
Physical and Mental Efforts
Prolong standing and walking required. Ability to lift up to 50 pounds and move patients. Required working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and meet patient/family psycho social needs. Requires hand-eye coordination and manual dexterity.
Essential Functions and Evaluation
Under the direction of the RN, participate in planning and implementing specialized procedures and duties that assist in meeting the nursing care needs of the patient in accordance with agency polices.

Completes appropriate documentation i.e. consent of services,
Completes appropriate documentation (i.e. consent of services, advance directives, nursing notes case conference, etc), into CRADLEMRx, in a timely manner.
Provide care utilizing infection control measures that protect both staff and patient (OSHA).
Provide effective communication to patient/family, team members, and other health care professionals.
Monitor assigned cases to ensure compliance with requirements to third party prayer.
Demonstrates commitment and professional growth by participating in in-service programs and competency.
